Coordinated healthcare is the core of PACE senior services. Each participant has a team of medical experts dedicated to providing personalized healthcare. Care is coordinated by the Gary and Mary West PACE interdisciplinary team.

Primary Medical and Nursing Care
Preventive and primary care is provided at the PACE center, as well as any necessary hospitalization and post-hospital care.
Participants do not need prior authorization to access qualified emergency services. Our clinical team and staff members are available 24 hours a day, 7 days a week. Our PACE clinic is open during normal business hours between 8 a.m. and 4:30 p.m PST.
Medical Specialty Care
Gary and Mary West PACE has an extensive provider network. The PACE senior healthcare plan ensures participants have full access with no additional cost to medically necessary items such as dentures, hearing aids and eyeglasses. If a participant’s primary care team determines that they would benefit from treatment from a GMWP network specialist, we will coordinate all appointments and procedures at no additional cost.
The medical care provided to PACE participants is all-inclusive and includes end-of-life and palliative care that can be provided at home or at a facility.
If your care team determines it’s medically necessary for additional in-home help, GMWP participants may receive in-home medical and personal care.
Our team will work to ensure the best level or medication management, meaning no missed doses and the right medications at the right time. All prescriptions, as well as prescribed over-the-counter medications, are included with PACE at no extra cost. Medication will be delivered directly to participant’s homes by our pharmacy and via USPS.
A registered dietitian assesses each participant’s nutritional needs, and recommendations are part of their individualized care plan. Nutritional counseling also addresses a variety of health issues, including weight loss and diabetes.
Physical, Occupational and Speech Therapy
Participants are assessed concerning their ability to complete various tasks and activity goals, and then prescribed a therapy program to meet their needs when deemed medically necessary. There are no limits to these programs, with each tailored to the individual needs and goals of the participant. Rehabilitative and restorative therapies are also provided by the PACE program.
Adult Day Center
Fun and engaging activities are designed and led by the GMWP Recreation Therapists. Activities at the center include bingo, game sessions, pet therapy, musical entertainment, and group outings.
GMWP provides transportation from your home to the PACE center and to all outside medical appointments.
Please note that GMWP participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from GMWP or a person or company authorized by GMWP. GMWP participants may be fully and personally liable for the costs of any out-of-network services.
