We understand that many seniors prefer the safety, comfort and familiarity of their own home. They want to remain as independent as possible, living in their own place rather than a nursing home. PACE makes this want possible; our service provides in-home nursing care to seniors in the San Marcos, San Diego area. This includes a holistic level of care at home, free transportation when needed, medication management, personal care, rehabilitative services and more.

Our program gives assistance in both personal health care and daily activities. After a home safety evaluation has taken place, and any necessary changes are made, our skilled nursing and qualified, caring staff assist our clients to go about their daily lives as independently as possible.

The PACE Home Safety Evaluation

We prioritize the safety of our participants above all else. For this reason, our qualified team conducts a mandatory safety evaluation, paramount for any at home rehabilitation services.

On joining the program, our nurse assesses any noticeable safety risks and tripping hazards These may include anything from loose rugs, electrical cords or clutter.

Our team might also suggest and indicate improvements that could make the home safer and more accessible for the potential participant. This may include wheelchair ramps or grab bars, and other in-home enhancements that are considered medically necessary.

These improvements are provided at no cost to our participants.

Types of Assistance Offered with In-Home Senior Care

Our in-home nursing care is ideal for seniors who require full time assistance but find it difficult to be away from the comfort of their home. Our participants can continue living as independently as possible, all while receiving the very best care and rehabilitative services at their doorstep.

Personal Assistance

Sometimes personal care assistance in the participant’s home is an essential part of retaining independence. Gary and Mary West PACE can provide in-home skilled nursing care for participants in San Diego when deemed medically necessary by our interdisciplinary team.

Once a participant’s need is validated, PACE can provide personal care in the privacy of their own home, with home health, personal care staff, and the provision of any necessary equipment and supplies.

The interdisciplinary team consists of:

  • Registered nurse
  • Dietician
  • Physician
  • Occupational therapist
  • Speech therapist
  • Social worker, and more.

Assistance with Daily Living

At Gary and Mary West PACE, our dedicated team strives to improve the quality of daily life with our at home services. We understand that every need is different and may require a personalized plan to keep up with the changing needs of our senior participants.

Our caring team of nurses and staff will assist you with all daily living activities that require medical attention and otherwise. These include, but aren’t limited to:

  • Medication management
  • Incontinence care
  • Dentistry, ophthalmology, and hearing care
  • In-home injections and blood draws
  • Support after hospitalization
  • Medication reminders
  • Nutrition planning
  • Bathing and dressing

Please note that GMWP participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from GMWP or a person or company authorized by GMWP. GMWP participants may be fully and personally liable for the costs of any out-of-network services.
