We understand that many seniors prefer the safety, comfort and familiarity of their own home. They want to remain as independent as possible, living in their own place rather than an nursing home. PACE at home makes this want possible. Our service provides in-home nursing care to seniors in the San Marcos, San Diego area.

Our program gives assistance in both personal health care and daily activities. After a home safety evaluation has taken place, and any necessary changes are made, our skilled nursing and qualified, caring staff assist our clients to go about their daily lives as independently as possible.

The PACE Home Safety Evaluation

One of the first steps in joining the program is a home safety evaluation with our nurse to assess fall risks and tripping hazards. These hazards may include loose rugs, electrical cords or clutter.

Our team might also suggest and indicate improvements that could make the home safer and more accessible for the potential participant. This may include wheelchair ramps or grab bars, and other in-home enhancements that are considered medically necessary.

These improvements are provided at no cost to participants as they are part of the PACE at home all-inclusive care.

Types of Assistance Offered with In-Home Senior Care

Our in-home nursing care provides seniors with the assistance they need to continue living as independently as possible.

Personal Assistance

Sometimes personal care assistance in the participant’s home is an essential part of retaining independence. Gary and Mary West PACE can provide in-home skilled nursing and health care for participants when deemed medically necessary by the PACE at home interdisciplinary team.

Once a need is validated, PACE can provide personal care in the privacy of your own home, with home health, personal care staff and the provision of equipment and supplies.

Assistance with Daily Living

Our qualified, friendly care staff will assist with daily living activities. These may include:

  • Medication management
  • Incontinence care
  • In-home injections and blood draws
  • Support after hospitalization
  • Medication reminders
  • Bathing and dressing
  • Light meal preparation
  • Light housekeeping

Please note that GMWP participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from GMWP or a person or company authorized by GMWP. GMWP participants may be fully and personally liable for the costs of any out-of-network services.
